| Background: The treatment of keloids remains a challenge. The wide array of therapeutic options available makes it challenging for physicians to choose a single treatment. A combination of modalities may be a good option. Objective: To study the therapeutic outcome of injections of triple combination (triamcinolone acetonide, 5-fluorouracil, and hyaluronidase) in the management of keloids and to assess the treatment outcome using dermatoscopy and the patient observer scar assessment scale (POSAS). Materials and Methods: In this prospective observational study, 16 keloids were treated with cryotherapy followed by injection of a triple combination (0.4ml of Injection triamcinolone acetonide 40mg/ml + 0.6ml of injection 5 fluorouracil 250mg/5ml + injection hyaluronidase 1500 U/ml). Each patient received 4 sessions at monthly intervals, and at each visit, clinical examination, dermoscopy, and POSAS were performed. Results: There was a statistically significant reduction in the height of the keloid (4.4mm) as well as the objective and subjective parts of the POSAS. There was a mean decrease of 24.5 points in the total POSAS score; the subjective score dropped by 49%, and the objective score decreased by 30% after the fourth injection. Following treatment, on dermoscopy, the persistent thicker areas of the scar showed the persistence of blood vessels. Conclusion: Injection triple combination with cryotherapy is an effective treatment modality for treating keloids. |
